Kistik Ekinokokkoz Tanısında İndirekt Hemaglütinasyon Testinin Kullanılması: İki Yıllık Retrospektif Laboratuvar Deneyimi

Çalışmamızda, Ocak 2020 ile Aralık 2021 tarihleri arasında laboratuvarımıza serum örneği gönderilen hastaların KE İndirekt Hemaglütinasyon Testi (IHA) sonuçlarının geriye dönük olarak değerlendirilmesi amaçlanmıştır.
YÖNTEM ve GEREÇLER: Laboratuvarımızda KE tanısı için ticari bir kit (Hydatidose, Fumouze laboratoires, Fransa) kullanılmaktadır. Çalışmamızda, hastaların demografik bilgileri, klinik bulguları, seroloji ve radyoloji sonuçları birlikte değerlendirilmiştir.
BULGULAR: Hastaların %14.12’sinin antikor titresi 1/320 ve üzerinde bulunmuştur. İndirekt Hemaglütinasyon testinde pozitiflik saptanan 75 hastanın 73’ünün radyolojisinde kist saptanmış olup, 58’inin kistinde KE’ye özgü bulgular olduğu belirtilmiştir. Seropozitiflik erkeklerde (%14.81) kadınlara (%13.65) göre daha yüksek saptanmıştır. En yüksek pozitiflik yüzdesi 20-29 yaş grubundadır. Kistlerin çoğunluğunun yerleşim yeri karaciğerdir (%87.67) ve yedi hastada çoklu organ kisti saptanmıştır. IHA’da antikor titresi 1/80-1/160 saptanan 43 hastanın 36’sının radyolojisinde kist saptanmış olup, bu kistlerin 28’inin KE’ye özgü bulguları olduğu belirtilmiştir.
TARTIŞMA ve SONUÇ: Çalışmamızda saptanan seropozitiflik yüzdesi ülkemizdeki diğer merkezlerden bildirilen sonuçlarla uyumlu bulunmuştur. Eşik değerin altında antikor titresine sahip hastaların radyolojik görüntülemelerinde KE ile uyumlu kistlerin varlığı, bu değerlerin sonuç raporunda verilmesinin hastaların tanı ve takibinde yararlı olacağını düşündürmektedir.
Use of Indirect Hemagglutination Test in the Diagnosis of Cystic Echinococcosis: Two Years of Retrospective Laboratory Experience

In our study, it was aimed to retrospectively evaluate the CE Indirect Hemagglutination Test (IHA) results of patients whose serum samples were sent to our laboratory between January 2020 and December 2021.
METHODS: A commercial kit (Hydatidose, Fumouze laboratoires, France) is used for the diagnosis of CE in our laboratory. Demographic information, clinical findings, serology and radiology results of the patients were evaluated together.
RESULTS: The antibody titer of 14.12% of the patients was found to be 1/320 and above. Cysts were detected in the radiology of 73 of 75 patients with positive IHA test, and CE-specific findings were reported in 58 of them. Seropositivity was found to be higher in males (14.81%) than females (13.65%). The highest percentage of positivity was in the 20-29 age group. Most of the cysts were localized in the liver (87.67%) and multi-organ cysts were detected in seven patients. Cysts were detected in the radiology of 36 of 43 patients with an antibody titer of 1/80-1/160 in IHA, and 28 of these cysts were reported to have CE-specific findings.
DISCUSSION AND CONCLUSION: The percentage of seropositivity detected in our study was found to be compatible with the results reported from other centers in our country. The presence of cysts compatible with CE in the radiological imaging of patients with antibody titers below the threshold value suggests that it would be useful to provide these values in the final report for the diagnosis and follow-up.
