New Enemy of Refugees: Tuberculosis

INTRODUCTION: Tuberculosis (TB) remains a significant threat to public health in Turkey. The disease can cause respiratory failure and is followed up in intensive care units (ICU) with different manifestations such as meningitis. The aim of this study was to present the results of a one-year TB follow-up.
METHODS: In this retrospective, cross-sectional study, evaluation was made of the data obtained from the files of patients who were hospitalized between January 1, 2018 and December 31, 2018 in the Anesthesia and Reanimation Clinic of Dr. Sadi Konuk Training and Research Hospital. After obtaining approval from the Ethics Committee, prognostic scores, reasons for hospitalization, presence of additional diseases, type of discharge, presence of hospital infections, duration of mechanical ventilation, types of diagnosis and culture results were recorded.
RESULTS: Information was retrieved from the files of 22 patients with tuberculosis. Reasons for admittance were respiratory failure in 12 patients (54.5%), septic shock in 6 (27.3%), and meningitis in 4 (18.2%). Of these 22 patients, 16 (72.7%) died and 6 (27.3%) were discharged with recovery and 40% of all patients were immigrants. The duration of invasive mechanical ventilation was statistically significantly longer in the survivor group (p: 0.024).
DISCUSSION AND CONCLUSION: Tuberculosis is still a serious disease in our country and it is estimated that the cases will be increased due to the increased migration rates in recent years. During the course of disease intensive care follow-up may be required due to the respiratory distress. Immigration and changing public health can cause structural changes in intensive care units.

GİRİŞ ve AMAÇ: Tüberküloz (TB), ülkemizde toplum sağlığını tehdit eden bir hastalık olarak önemini korumaktadır. Hastalık solunum yetmezliğine yol açabildiği gibi menenjit gibi farklı klinik tablolar ile yoğun bakım ünitelerinde takip edilmektedir. Çalışmamızın amacı bir yıllık TB takiplerimiz paylaşmayı hedefledik.
YÖNTEM ve GEREÇLER: Bu çalışma Bakırköy Dr. Sadi Konuk Eğitim ve Araştırma Hastanesi Anestezi ve Reanimasyon kliniğinde 1 Ocak 2018 ile 31 Aralık 2018 tarihleri arasında yatışı yapılan hastalar dosyalarından retrospektif kesitsel planlanmıştır. Etik kurul onayı alındıktan sonra prognostik skorları, yatış nedenleri, ek hastalık varlığı, taburculuk şekilleri, hastane infeksiyonları varlığı, mekanik ventilasyon süreleri, tanı şekli, kültür sonuçları kaydedilmiştir. Tüm hastalar dörtlü antitüberkuloz tedavi almıştır
BULGULAR: Çalışmamızda tüberküloz tanılı 22 hasta tespit edilmiştir. 12 hasta (%54,5) solunum yetmezliği, 6 hasta (%27,3) septik şok, 4 hasta (%18,2) menenjit sebebi ile kabul edilmiştir. Hastaların 16’sı (%72,7) kaybedilirken, 6’sı (%27,3) şifa ile taburcu edilmiştir. İnvaziv mekanik ventilasyon süresi sağ kalan grupta istatistiksel anlamlı uzun bulunmuştur (p: 0,024).
TARTIŞMA ve SONUÇ: Hastalığın doğası gereği solunum sıkıntısı sonucu mekanik ventilasyona ihtiyacı nedeniyle yoğun bakım ünitelerinde takip edilmeleri gerekebilir.
