Çeşitli risk faktörlerinin kolaylaştırıcı etkisiyle Fusarium keratiti hızla ilerleyip kornea tahribatına ve endoftalmiye yol açabilir. Bu olguda, 50 yaşında erkek hasta, tarlada çalışırken gözüne mısır püskülü girmesi sonrası sol gözde ağrı ve görmede azalma gibi şikayetlerle bir göz kliniğine başvurmuş ancak verilen tedaviye rağmen şikayetlerinin gerilememesi üzerine üniversite hastanesi göz hastalıkları kliniğine yönlendirilmiştir. Sol gözde konjuktival hiperemi ve korneal opasite bulanan hastanın yapılan biyomikroskopik ön segment muayenesinde korneada inferior parasantral yerleşimli 5x5 mm genişlikte epitel defekti ve tüysü kenarlı sarı-beyaz renkli stromal infiltrat olduğu görülmesi üzerine mantar enfeksiyonu düşünülmüştür. Kültür ekimleri sonrasında üreyen küf kolonileri incelenmiş ve laktofenol pamuk mavisi boyalı bakıda görülen yapıların Fusarium spp. ile uyumlu olduğu görülmüştür. İzole edilen suş, dizi analizi yöntemi ile tür düzeyinde tanımlanmış ve etkenin ‘Fusarium solani tür kompleksi’ olduğu belirlenmiştir. Uygulanan tüm medikal tedavilere ve cerrahi müdahalelere rağmen gözün iç yapılara ilerlemiş olan enfeksiyon kontrol altına alınamamış ve evisserasyon ameliyatı yapılmıştır. Evisserasyon sonrası takiplerde herhangi bir enfeksiyon bulgusu ile karşılaşılmamıştır. Fusarium solani, literatürde antifungal direnci yüksek bir etken olarak bildirilmektedir. Nitekim bu olguda, sağlık kuruluşlarına başvurmada muhtemel gecikme ve Fusarium solani gibi dirençli bir küf mantarının etken olması; klinik bulguların ciddi seyretmesine ve uygulanan tedavi protokollerine rağmen endoftalmi sonucu gözün kaybedilmesine neden olmuştur. Bu olgunun, etkenin bulaş şekli ve klinik seyri açısından dikkate değer olduğu düşünülmektedir. Fusarium keratitis may permeate rapidly with the facilitating effect of various risk factors and corneal destruction and endophthalmitis may develop eventually. In this report, we present a 50-year-old male patient, whose left eye was traumatized by a corn tassel while working in the farm and applied to an ophthalmology clinic with complaints of pain and decreased vision. As his complaints continued despite treatment, he was referred to ophthalmology clinic of a university hospital. Corneal opacity and conjunctival hyperemia were observed in the left eye. Possibility of a fungal infection was considered due to the presence of a 5x5 mm epithelial defect and a white stromal infiltrate with feathery edges in the inferior paracentral part of the cornea during biomicroscopic examination. Mold colonies grew in culture and were interpreted as Fusarium spp. with lactophenol cotton blue staining. The isolated strain was identified by sequence analysis, and the causative agent was determined as ‘Fusarium solani species complex’. Despite medical treatment and surgical operations, the infection spread to the intraocular structures, which obliged the evisceration of the eye. No signs of infection were found in the postoperative follow-up examinations. It is thought that this case report may contribute to the literature in terms of drawing attention to the transmission routes and clinical course of the causative agent.
