İÜ-C Cerrahpaşa Tıp Fakültesi Hastanesi’ne Başvuran Hastalarda 2013-2018 Yılları Arasında Toxoplasma gondii, Rubella ve Sitomegalovirus Seropozitifliği ile Avidite Test Sonuçlarının Retrospektif Olarak Değerlendirilmesi

GİRİŞ ve AMAÇ: Bu çalışmada Cerrahpaşa Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Seroloji/ELISA Laboratuarı’na Ocak 2013-Aralık 2018 yılları arasında çeşitli klinik endikasyonlar nedeniyle gönderilen Toxoplasma gondii, rubella ve Cytomegalovirus (CMV) enfeksiyonu şüpheli yada gebelik izlemiyle ilgili rutin kontrol amaçlı olguların serum örneklerinde IgM ve IgG antikor seropozitifliği ile IgG avidite test sonuçlarının retrospektif olarak değerlendirilmesi amaçlandı.
YÖNTEM ve GEREÇLER: 2013-2018 yılları arasında İÜ-C Cerrahpaşa Tıp Fakültesi Hastanesi’ne başvuran 84.807 hastaya ait serum örneklerinde immun capture ELISA yöntemi ile çalışılmış T.gondii, rubella ve CMV IgM antikorları ve indirekt ELISA yöntemiyle saptanan T.gondii, rubella ve CMV IgG antikorları değerlendirildi. Ayrıca indirekt ELISA yöntemi ile T.gondii, rubella ve CMV IgG avidite test incelemesi yapılan 916 olgunun IgG avidite test sonuçları irdelendi
BULGULAR: Çalışmamızda T. gondii, rubella ve CMV için IgM/IgG pozitifliği sırasıyla%2,2/%97,8, %0,24/%99,7 ve %3,1/%94 olarak saptanmıştır. IgM ve IgG pozitif olan, her üç etkene dönük avidite indeks testi istenen olgu sayısı 274 (%30) olup, T.gondii, rubella ve CMV için düşük avidite indeksi saptanma oranı sırasıyla; 44 (%16), 9(%3,2) ve 8(%2,9)’dir.
TARTIŞMA ve SONUÇ: Retrospektif değerlendirme sonucunda elde ettiğimiz her üç etkenle ilişkili seropozitiflik oranlarının ülkemizde yapılmış seroepidemiyolojik temelli çalışmalara ait veriler ile benzerlik gösterdiği belirlenmiştir. T. gondii, rubella ve CMV IgG avidite düşük saptama olgu sıklığı hem popülasyonda etkene dönük reaktivasyon/ reenfeksiyon riski fazla olduğundan avidite testinin önemini tekrardan vurgulamıştır.
Retrospective Evaluation of Toxoplasma gondii, Rubella and Cytomegalovirus Seropositivity and Avidity Test Results in patients admitted to İU-C Cerrahpaşa Medical Faculty Hospital Between 2013-2018

INTRODUCTION: Aim of this study was to evaluate retrospectively IgM and IgG antibody seropositivity and IgG avidity test results in serum samples cases of suspected Toxoplasma gondii, rubella and Cytomegalovirus (CMV) infections or routine control for pregnancy follow up which sent to Cerrahpaşa Medical Faculty Medical Microbiology Serology/ELISA Laboratory between January 2013 and December 2018 due to various clinical indications
METHODS: T.gondii, rubella and CMV IgM antibodies examined by ımmunocapture ELISA method and T.gondii, rubella and CMV IgG antibodies detected by indirect ELISA method were evaluated in serum samples of 84,807 patients who applied to İU-C Cerrahpaşa Medical Faculty Hospital between 2013-2018.In addition, IgG avidity test results were examined in serum samples of T.gondii, rubella and CMV IgG avidity tests were performed by indirect ELISA of 916 cases.
RESULTS: In the study, IgM / IgG positivity was found to be 2.2% / 97.8%, 0.24% / 99.7% and 3.1%/ 94% for T. gondii, rubella and CMV, respectively. IgM and IgG positive, the number of cases avidity index tests for all three agents was 274 (30%). The rate of low avidity index for T.gondii, rubella and CMV were; 44 (16%), 9 (3.2%) and 8 (2.9%) respectively.
DISCUSSION AND CONCLUSION: As a result of the retrospective evaluation, the seropositivity rates associated with all three agents were similar of the seroepidemiological studies conducted in our country. The incidence of T. gondii, rubella and CMV IgG low avidity case reiterated the importance of avidity test both in terms of causative reactivation/re-infection risk in the population
