Başarılı PTCA Uygulaması Öncesi ve Sonrasında Ventriküler Aritmilerin Değerlendirilmesi

Kardiyoloji Enstitüsü Haseki, İstanbulKoroner anjiyoplastinin (PTCA) ventriküler aritmi sıklığı üzerine etkisini incelemek amacıyla, başarılı PTCA uygulanan 24 hastanın PTCA öncesi 1-15 gün içinde, PTCA hemen sonrası ve 15. gününde 24 saatlik Holter monitoring kayıtları alındı. Tek ventriküler erken atım (VEA) ve total VEA sayısı, ikili (couplet) VEA, bigemine-trigemine VEA, idioventriküler ritm ve ventriküler taşikardi (VT) saptanan hasta sayısı açısından karşılaş¬tırıldı. 24 saatlik tek VEA sayısı ortalama değeri PTCA öncesi 202±512, PTCA hemen sonrası 317±730, 15. gün 285±760 (p:ns), total VEA sayısı ortalama değeri PTCA öncesi 244±571, hemen sonrası 344±794, 15. gün 303± 832 bulundu (p:ns) bigemine-trigemine VEA, idioventriküler ritm ve ventriküler taşikardi bulunan hasta sayısı açısından yapılan karşılaştırmada da üç Holter kaydı arasında anlamlı fark saptanmadı. Sonuç olarak PTCA uygulamasını takibeden ilk 24 saat içinde daha belirgin olmak üzere PTCA sonrası, tek ve total VEA'larda artış görüldü ancak istatistiksel olarak anlamlı bulunmadı. PTCA uygulamasının ventriküler aritmilerde önemli değişikliğe neden olmadığı kararına varıldı.

Y.Vedat AYTEKİN1, Muzaffer ÖZTÜRK1, İnci FIRATLI1, Servet ÖZTÜRK1, Cem DEMİROĞLU1To evaluate the effect of successful percutaneous transluminal coronary angioplasty (PTCA) on ventricular arrhythmias, ambulatory 24-hour holter recordings were taken 1 to 15 days before PTCA, soon after and 15 days after the procedure. These three groups of recordings were compared to each other with respect to the number of single ventricular premature beats (VPBs) and total VPBs, and the number of patients having couplet VPBs, bigeminytrigeminy, idioventricular rhythm and ventricular tachycardia. The mean value of single VPBs detected was 202±512 before PTCA, 317±730 soon after and 285±760 15 days after PTCA (p:ns), the mean value of total VPBs was 244±571 before PTCA, 344;t794 soon after and 303±832 15 days after PTCA (p:ns). There were also no significant difference between these 3 groups of recordings with respect to the number of patients with total VPBs, couplet VPBs, bigeminy-trigeminy, idioventricular rhythm and ventricular tachycardia. Although it was not statistically significant, the mean values of single and total VPBs were increased after PTCA. The rise in this values were more striking soon after PTCA. In conclusion, PTCA does not affect ventricular arrhythmias significantly.
