Derlemeler Otonom Sinir Sistemi ve AritmilerM. Bülent ÖZİN1Başkent Üniversitesi Tıp Fakültesi, AnkaraHem supraventriküler, hem de ventriküler aritmilerin oluşumunda, aritmiye zemin hazırlayan yapısal ortam ve tetikleyici faktörler kadar organizmanın o sırada içinde bulunduğu otonom denge de etkilidir. Her nekadar çeşitli deneysel çalışmalarda otonom sinir sisteminin bu rolü detaylı olarak gösterilmişse de klinikte gözlenen aritmilerde, otonom sistemin durumunu belirleyebilecek basit ve hassas testlerin mevcut olmamasından dolayı, otonom etkiler gizli kalabilmektedir. Taşiaritmilerin oluşumundan sorumlu tutulan mekanizmalar olan anormal otomatisite, tetiklenmiş aktivite ve reentrinin oluşumunda özellikle sempatik sinir sisteminin rolü büyüktür. Otonom sinir sistemi tarafından yoğun olarak inerve edilen sinüs ve atriyoventriküler düğümün aritminin vazgeçilmez bir parçası olarak yer aldığı birçok ritm bozukluğunda hem sempatik hem de parasempatik sinir sisteminin aritmilerin oluşumu, devamı, sonlanması ve önlenmesinde büyük etkinliği vardır. Ayrıca yapısal kalp hastalığı olan ya da olmayan bireylerde gelişen ciddi ventriküler aritmilerde ve ani ölümde otonom sinir sisteminin etkileri çeşitli tekniklerle gösterilmiştir. Otonom modülasyonun aritmilerin tedavisindeki etkinliği de özellikle beta blokerlerin yaygın kullanımıyla klinik uygulamaya da yansımıştır. Bülent ÖZİN1Autonomic tones is an essential factor in the genesis of both supraventricular and ventricular arrhythmias as well as the substrate for the arrhythmia and the triggering factors. Although the role of the autonomic nervous system has been well established in many elegant experimental studies, these effects may not seem to be obvious in many elinical arrhythmias due to lack of simple and sensitive tests to evaluate the autonomic nervous system. Autonomic nervous system, especially the sympathetic arm is involved in all of the proposed mechanisms in the genesis of tachyarrhythmias (abnormal automaticity, triggered activity and reentry). Sympathetic and parasympathetic nervous system play an essential role in the genesis, perpetuation, termination and prevention of arrhythmias where the parts of the heart, richly innervated by the autonomic nervous system such as the sinus node and the atrioventricular node are involved. Moreover, the role of the autonomic nervous system has been shown in various ventricular arrhythmias and sudden cardiac death. The unique antiarrhythmic action of beta blockers reflect the therapeutic efficacy of autonomic modulation.
