Erişkinlerde Açık Duktus Arteriozusun Perkütan Transkateter Yöntemle Kapatılması

Patent duktus arteriozusun (PDA) perkütan transkateter yöntemle kapatılması bugün cerrahi tedaviye alternatif bir tedavi seçeneği haline gelmiştir. Bu çalışmada Anabilim Dalımızdaki erişkin olgulardaki sonuçlarımız sunulmaktadır. 1992 yılından beri 9 olguda Rashkind PDA oklüzyon sistemi kullanılarak transkateter oklüzyon uygulanmıştır. Olguların 8'i kadın, 1'i erkek olup yaşları 17-59 (27±13) arasında idi. PDA çapı 7.87 ± 1.62 mm (5.7-10.1 mm) arasında değişmekte idi. Qp/Qs 1.78±0.19 olarak hesaplandı. 1 olgu daha önceden aynı tanı ile opere edilmişti. Tüm olgularda 17 mm çapında şemsiye kullanılmıştır. İmplantasyon olguların tümünde başarılı olmuştur (% 100). Tam oklüzyon 1. haftada 4 olguda, 1. yılda 7 olguda elde edilmiştir. İnkomplet oklüzyon olan 2 olguda 2. kez oklüzyon denenmiş, 1 olguda başarılı olunmuş ve tam oklüzyon elde edilmiştir. Böylece tam oklüzyon 9 olgunun 8'inde (% 88.8) elde edilmiştir. Rezidüel şantı olan olguda Qp/Qs 1.31 idi. Hiçbir olguda major komplikasyon görülmemiştir. Hernekadar olgu serimiz küçük de olsa bu çalışma göstermektedir ki açık duktus arteriozusun transkateter yöntemle kapatılması erişkinlerde de oldukça güvenli ve başarılı bir tedavi seçeneğidir.

Percutaneous transcatheter closure of the persistently patent ductus arteriosus (PDA) with the Rashkind PDA occluder system is an established effective therapeutic modality in the majority of children with PDA. We report here our results with adult patients with PDA. The study comprised 9 patients (8 females, 1 male with a mean age of 27±13, range 17-59. The diameter of the PDA ranged from 5.7 to 10.1 mm (7.8±1.6). Qp/Qs was calculated as 1.8±0.2 with the Fick method before the procedure. A 17 mm-device was used in all patients. Complete closure was accomplished in 4 patients at 1 week, 7 patients in 1 year. One of the 2 attempted reocclusions was successful (with the 17 mm-device). As a result, complete occlusion of the PDA with the Rashkind device was 89% (8 of 9 patients) in this study. No device embolization or major complictaions occurred during the procedure on the follow-up period. Percutaneous transcatheter PDA occlusion using the Rashkind occluder device is a feasible nonsurgical method also in adult patients with PDA.
