Surgical Management of Corneal Hydrops: Case Series
Gökçen Özcan, Ömür Özlenen UçakhanAnkara University, Faculty of Medicine, AnkaraBackground: Descemet’s membrane (DM) rupture/detachments have traditionally been treated conservatively, with limited efficacy, and a long rehabilitation period significantly affecting patients’ vision and quality of life. Although there are no established gold standards for the timing and nature of the treatment, we aim to highlight the importance of the current optimal intervention methods.
Case: We herein report 4 cases of DM detachment. First case with keratoglobus and second case with keratoconus had acute hydrops and were treated with isoexpansile 14% C3F8 injection to anterior chamber. Third case with keratoglobus had chronic hydrops complicated with multiple stromal clefts on anterior segment optical coherent tomography and fourth case had a chronic broad DM detachment which occurred after cataract surgery, both treated with intracameral C3F8 injection together with corneal compressive sutures. In all four cases, DM reattached completely and effectively with surgical interventions.
Conclusions: Surgical management of DM rupture/detachment with intracameral gas injection and compressive corneal sutures seems to provide fast symptomatic relief and less healing-related corneal scarring with better visual rehabilitation and may alleviate the need for corneal transplant surgery in this group of patients.
Korneal Hidropsun Cerrahi Tedavisi: Vaka Serisi

Descemet membran (DM) rüptürü / dekolmanı konservatif olarak, sonrasında hastanın görme ve yaşam kalitesini önemli ölçüde etkileyen uzun bir rehabilitasyon dönemi ile sınırlı etkililikle tedavi edilmektedir. Bu olgu serisi ile, tedavinin zamanlaması ve doğası için belirlenmiş altın standart tedavi olmamasına rağmen, mevcut optimal müdahale yöntemlerinin öneminin vurgulaması amaçlanmıştır.
Olgu serisi
Bu olgu serisinde dört DM dekolmanı vakası bildirmekteyiz. İlk olgu keratoglobus, ikinci olgu ise keratokonus nedeni ile akut hidrops geçirmişti ve ön kamaraya izo-ekspansil % 14’lük C3F8 enjeksiyonu ile tedavi edildi. Üçüncü olgu keratoglobus nedeni ile hidrops geçirmiş ve kronikleşmiş ön segment optik koherens tomografide çoklu stromal yarıklarla komplike olmuştu ve dördüncü olguda katarakt cerrahisi sonrası kronik geniş DM dekolmanı gelişmişti, her ikisi de korneal kompresif sütürlerle birlikte intrakamaral % 14’lük C3F8 enjeksiyonu ile tedavi edildi. Dört vakanın hepsinde, DM cerrahi müdahale ile tamamen ve etkili bir şekilde yatıştırıldı.
Sonuç
DM rüptürü / dekolmanının intrakamaral gaz enjeksiyonu ve kompresif korneal sütürler ile cerrahi tedavisi, hızlı semptomatik rahatlama, daha iyi görsel rehabilitasyon ile daha az kornea skarlaşma sağlanmaktadır ve bu hasta grubunda kornea nakli ihtiyacını azaltabilir.
