Vitrectomy Due to Vitreous Hemorrhage and Tractional Retinal Detachment Secondary to Eales’ Disease

Objectives: To investigate visual and anatomical outcomes of vitreoretinal surgeries in patients with Eales’ disease.
Materials and Methods: In this retrospective study, 22 eyes of 21 patients with vitreous hemorrhage (VH) or tractional retinal detachment (TRD) secondary to Eales’ disease who underwent vitreoretinal surgery between January 1997 and December 2015 and had at least one year follow-up, were included.
Results: The mean best corrected visual acuity (BCVA) was significantly higher at final visit (0.6 ± 0.9 logMAR) than the preoperative values (1.8 ± 1.1 logMAR) (p < 0.001). After surgery BCVA was stable in 4 eyes (18.2%), increased in 16 eyes (72.7%), and decreased in 2 eyes (9.1%). Although the mean BCVA was better in VH group (0.3 ± 0.34 logMAR) than retinal detachment group (0.9 ± 1.1 logMAR), the difference was not statistically significant (p = 0.1). Multivariable lineer regression analyses revealed that preoperative or postoperative proliferative vitreoretinopathy (PVR) C, preoperative retinal detachment involving the macula, postoperative neovascular glaucoma, and long preoperative duration of disease had a negative association while preoperative BCVA had a positive association with final BCVA. Preoperative retinal and disc neovascularization, rubeosis iridis, total posterior hyaloid detachment, preoperative retinal laser photocoagulation, indication of surgery, diameter of scleratomy (20 gauge / 23 gauge), preoperative lens status, preoperative or postoperative epimacular membrane, peroperative iatrogenic retinal breaks, postoperative hypotony, cystoid macular edema, and new or recurrent retinal detachment were not associated with final BCVA. The primary anatomic success rate was 81.8% and the final anatomic success rate was 90.9%.
Conclusion: In Eales' disease, good visual results can be obtained with vitreoretinal surgery if the detachment area does not affect the macula and preoperative or postoperative PVR C does not develop.

Eales Hastalığına Bağlı Gelişen Vitreus Hemorajisi ve Traksiyonel Retina Dekolmanında Vitrektomi

Amaç: Eales hastalığında uygulanan vitreoretinal cerrahilerin görsel ve anatomik sonuçlarını ve son görme keskinliğine etki eden faktörleri araştırmak.
Gereç Ve Yöntem: Retrospektif olarak tasarlanan bu çalışmaya Eales hastalığına bağlı vitreus hemorajisi (VH) veya traksiyonel retina dekolmanı (TRD) nedeniyle Ocak 1997 ve Aralık 2015 tarihleri arasında vitreoretinal cerrahi uygulanan, en az bir yıl takipli 21 hastanın 22 gözü dahil edildi.
Bulgular: En iyi düzeltilmiş görme keskinliği (EİDGK) ortalaması son muayenede (0.6 ± 0.9 logMAR) preoperatif döneme (1.8 ± 1.1 logMAR) göre anlamlı şekilde artmıştı (p < 0.001). Cerrahi sonrası EİDGK 4 gözde (%18.2) stabil kalırken, 16 gözde (%72.7) arttı, 2 gözde (%9.1) ise azaldı. VH grubunda son EİDGK ortalaması (0.3 ± 0.34 logMAR) retina dekolmanı grubuna (0.9 ± 1.1 logMAR) göre daha iyi olmasına karşın fark istatistiksel olarak anlamlı değildi (p = 0.1). Çok değişkenli lineer regresyon analizinde preoperatif veya postoperatif proliferatif vitreoretinopati (PVR) C, preoperatif makulayı etkileyen dekolman, postoperatif neovasküler glokom ve cerrahi öncesi uzamış hastalık süresi son EİDGK ile negatif ilişkili, preoperatif EİDGK ise pozitif ilişkili olarak bulundu. Preoperatif retinal neovaskülarizasyon, disk neovaskülarizasyonu, rubeozis iridis, total arka hyaloid dekolman, preoperatif retinal lazer fotokoagulasyon, cerrahi endikasyon, skleratomi çapı (20 gauge / 23 gauge), preoperatif lens durumu, preoperatif veya postoperatif epimaküler membran, peroperatif iyatrojenik retinal yırtık, postoperatif hipotoni, kistoid maküler ödem, ve yeni veya nüks retina dekolmanı son EİDGK ile ilişkili bulunmadı. Primer anatomik başarı %81.8, son anatomik başarı %90.9 idi.
Sonuç: Eales hastalığında dekolman alanı makulayı etkilemiyorsa ve preoperatif veya postoperatif PVR C gelişmemişse vitreoretinal cerrahiyle iyi görsel sonuçlar elde edilebilir.
