Donor Cornea Use in Scleral Surface Reconstruction

Aim: To investigate the indications and investigation of the treatment of scleral surface problems with donor corneal grafts that are not used in keratoplasty surgery or are left over from keratoplasty.
Method: Files of 16 cases using corneal tissue to close the scleral tissue defect and cover the exposed glaucoma drainage implant were evaluated retrospectively. Partial thickness grafts were prepared using a combined microkeratome system with artificial anterior chamber in 10 eyes, and using a combined microkeratome system, and manually by dissecting with blade in 3 eyes. Full thickness grafts were used in 3 eyes.
Results: Eight women and 8 men were between 5-79 (mean; 39.37 ± 24.68) years old. Indications for the use of corneal tissue on the scleral surface; limbal dermoid excision (n = 2), pterygium surgery (n = 1), intraocular lens removal and scleral fixation intraocular lens (SFIOL) implantation (n = 1), exposed SFIOL suture coverage (n = 1), trauma (n = 2), scleral tissue losses due to recurrent glaucoma surgeries (n = 5) and exposed glaucoma drainage implant (n = 4). Cases were followed up for 6-42 (mean; 14.37 ± 9.14) months. During the follow-up period, graft infection, thinning, immunological graft rejection and visual loss were not observed in any case. At final examination a good combination of graft-receiving tissue, and a smooth ocular surface and cosmetic appearance in all eyes was obtained.
Discussion: Donor corneas, which are not suitable for corneal transplantation or left over from the cornea transplant, can be used in scleral tissue loss due to various pathologies and in the treatment of glaucoma drainage implant erosion. In these cases, the use of corneal grafts provides a good ocular surface restoration and cosmetic appearance. It should be investigated that the method is effective and safe with large patient series and long follow-up times.

Amaç: Keratoplasti ameliyatında kullanılmayan veya keratoplastiden arta kalan donör kornea greftlerinin skleral yüzey problemlerinin tedavisinde kullanım endikasyonları ve sonuçlarını araştırmak.
Metod: Skleral doku defektini kapatmak ve açıkta kalan glokom drenaj implantının üzerini örtmek için korneal doku kullanılan 16 olgunun dosyaları retrospektif olarak incelendi. Parsiyel kalınlık greftler 10 gözde yapay ön kamara ile kombine mikrokeratom sistemi kullanılarak, 3 gözde bıçakla diseke edilerek manuel olarak hazırlandı. Üç gözde tam kalınlık greft kullanıldı.
Sonuçlar: Sekiz kadın, 8 erkek olgu 5-79 (ort; 39.37 ± 24.68) yaş arasında idi. Skleral yüzeyde korneal doku kullanımı için endikasyonlar; limbal dermoid eksizyonu (n=2), pterjiyum cerrahisi (n=1), IOL çıkarımı ve SFIOL implantasyonu (n=1), açıkta kalmış SFIOL sütürünün örtülmesi (n=1), travma (n=2), tekrarlayan glokom cerrahileri nedeniyle skleral doku kayıpları (n=5) ve açıkta kalmış glokom drenaj implantı (n=4) idi. Olgular 6-42 (ort; 14.37 ± 9.14) ay takip edildi. Takip süresi boyunca hiçbir olguda greft enfeksiyonu, incelmesi, immünolojik greft rejeksiyonu ve görme kaybı izlenmedi. Tektonik lamellar greft hiçbir olguda sonuç görme keskinliğini olumsuz etkilemedi. Sonuç muayenede tüm gözlerde greft alıcı doku ile iyi bir birleşme gösterdi, düzgün bir oküler yüzey ve kozmetik görünüm elde edildi.
Tartışma: Kornea nakli için uygun olmayan veya kornea naklinden arta kalan donör kornealar çeşitli patolojilerden kaynaklanan skleral doku kayıplarında ve glokom drenaj implant erezyonlarının tedavisinde kullanılabilir. Bu olgularda korneal greft kullanımı iyi bir oküler yüzey restorasyonu ve kozmetik görünüm sağlamaktadır. Yöntemin etkili ve emniyetli olduğu geniş hasta serileri ve uzun takip süreleri ile araştırılmalıdır.
