Evaluation of the Clinical Findings of the Cases with Penetrating Keratoplasty, Follow-up with Telephone Visits Due to the COVID-19 Pandemic

Purpose: To evaluate the clinical finding changes of the keratoplasty patients who could not be examined face-to-face and were followed up with telephone calls during the COVID-19 pandemic.
Methods: Patients with penetrating keratoplasty who showed upbetween March 2020 and February 2021 to cornea department were grouped according to progression in comparison to their last visits (Group 1 no progression, Group 2 progression). Patients’ last visit prior to COVID-19 pandemic and their first visits after the pandemic were evaluated. The demographic data, follow-up period, ophthalmological examination findings of all cases were recorded. The obtained data were compared between the groups.
Results: Thirty-five eyes of 35 patients were included in the study. Signs of progression were detected in 8 (22.8%) of the patients (Group 1), and no progression findings was detected in 27 (77.2%) of the patients (Group 2). In the last control prior to COVID-19 pandemic mean BCVA were 1.26 ± 0.43 (0.52-1.80) LogMAR in Group 1 and 1.41 ± 1.02 (0-3.1) LogMAR in Group 2 (p = 0.692). The mean BCVA in the last control during pandemic were 2.07 ± 0.86 (1.3-3.1) in Group 1 and 1.49 ± 1.08 (0-3.1) LogMAR in Group 2 (p = 0.08). In the last control during COVID-19 pandemic, the mean intraocular pressure of Group 1 was 16.38 ± 8.58 (0-31) mmHg, and Group 2 was 17.11 ± 3.7 (11-26) mmHg (p = 0.984).
Conclusion: The continuation of the treatment initiated prior to the pandemic was probably the most important reason why no worsening was observed in keratoplasty patients. In cases where face-to-face visits of the patients can be disrupted such as a pandemic, it may be possible to follow the patients safely with telemedicine visits until the challenging conditions disappear.

Amaç: COVID-19 pandemisi süresince yüz yüze kontrolleri yapılamayan ve telefon vizitleri ile takip edilen kornea nakilli hastaların klinik bulgularındaki değişimin değerlendirilmesi.
Gereç ve Yöntem: Kliniğimize Mart 2020 – Şubat 2021 tarihleri arasında başvuran penetran keratoplastili olgular, önceki durumlarına göre kötüleşme varlığına göre gruplandırıldı (Grup 1 progresyon yok, Grup 2 prograsyon var). COVID-19 pandemisinden önceki ve COVID-19 pandemisi kapanma sonrası ilk kontroldeki oftalmolojik muayene bulguları değerlendirmeye alındı. Tüm hastaların demografik verileri, takip süreleri ve oftalmolojik muayene bulguları not edildi. Elde edilen veriler gruplar arasında karşılaştırıldı.
Bulgular: Çalışmaya 35 hastanın 35 gözü dahil edildi. Çalışmaya dahil edilen hastalardan 8’inde (%22,8) kötüleşme bulguları saptanırken diğer olguların durumları stabildi. Grup 1’de COVID-19 pandemisi öncesi son kontroldeki EİDGK 1,26±0,43 (0,52–1,80) LogMAR iken Grup 2’de 1,41±1,02 (0-3,1) idi (p=0,692). Grup 1’de COVID-19 pandemisi döneminde yapılan son kontroldeki EİDGK 2,07±0,86 (1,3-3,1) LogMAR, Grup 2’de 1,49±1,08 (0-3,1) idi (p=0,08). COVID-19 pandemisi döneminde ise Grup 1’in ortalama göz içi basıncı 16,38 ± 8,58 (0-31) mmHg, Grup 2’nin ortalama göz içi basıncı 17,11 ± 3,7 (11-26) mmHg idi (p=0,984).
Tartışma: Çalışmamıza dahil edilen olguların pandemi öncesi başlanan tedavilerine devam etmesi kötüleşmenin izlenmemesinin muhtemelen en önemli nedenini oluşturmuştur. Pandemi gibi hastaların yüz yüze ziyaretlerinin aksayabildiği durumlarda hastalarla yapılacak tele tıp vizitleri ile zorlu koşullar ortadan kalkana kadar hastaların güvenli takibi mümkün olabilir.
