Amaç: Kronik bel ağrılı hastalarda egzersiz ve egzersiz ile beraber fizik tedavinin etkinliklerini araştırmak ve birbiriyle karşılaştırmak Gereç ve Yöntem: 3 aydan uzun süredir mekanik bel ağrısı olan 23’er hasta polikliniğe başvuru sırasına göre egzersiz ya da fizik tedavi+egzersiz gruplarından birine dahil edildi. İki grup da 14 gün boyunca, günde 2 seans lomber fleksiyon+ekstansiyon, lomber kasları ve karın kaslarını güçlendirme ve iliopsoas, hamstring ve kuadrisepsleri germe egzersizlerini yaptı. Ayrıca fizik tedavi grubuna 10 seans sıcak paket+terapötik ultrason+interferansiyel akım tedavisi verildi. Bel ağrısı şiddeti görsel analog skala (VAS) ile, bel hareket açıklığı el parmak-zemin mesafesi(EPZM) ve modifiye Schober testi ile, fonksiyonel durum modifiye Oswestry bel ağrısı skalası ile, yaşam kalitesi Short form-36 (SF-36) ile tedavi öncesi ve tedaviden 1 ay sonra değerlendirildi. Bulgular: Egzersiz grubu yaş ortalaması 59, 21 kadın, 2 erkek hastadan oluşuyordu. Fizik tedavi grubunda yaş ortalaması 60 olan 20 kadın 3 erkek hasta vardı. Her iki grupta da tedavi sonrası ağrı azaldı, EPZM ve M.Schober ölçümü arttı, fonksiyonel durum iyileşti ve SF-36 kategorilerinden ağrı ve fiziksel fonksiyonlarda iyileşme oldu. Egzersiz grubunda SF36- fiziksel rol güçlüğü de iyileşti. Fizik tedavi grubunda ağrıda azalma, EPZM’de artma, fonksiyonel durumdaki iyileşme anlamlı derecede fazla idi. Modifiye Schober testi ve yaşam kalitesindeki değişimler açısından gruplar arasında fark bulunmadı. Sonuç: Sonuç olarak kronik bel ağrısında egzersizler de egzersiz+fizik tedavi de etkindir. Egzersiz tedavisinden yeterince fayda görmeyen hastalara fizik tedavi uygulamaları ile başarılı sonuçlar alınabilir. Aim: Investigating and comparing the effects of exercise and physical therapy accompanying exercise treatments in patients with chronic low back pain Method: Twenty three patients with mechanichal type low back more than 3 months were included one of the exercise or the physical therapy+exercises groups according to their application sequence. Both of the groups performed lumbar flexion and extension exercises, strengthening of the lumbar and abdominal muscle exercises and iliopsoas, hamstring and quadriceps strecthing exercises two times a day for fourteen days. The physical therapy group were given hot pack+ therapeutic ultrasound+ interferential current for ten days additionaly. Degree of low back pain was evaluated with visual analog scale(VAS), range of joint motion was evaluated with hand finger floor distance(HFFD) and modified Schober test, functional status was evaluated with modified Oswestry low back pain scale and quality of life was evaluated with Short form-36 (SF-36) before and a month after the treatments. Results: In both groups (exercise group: mean age 59, 21 female, 2 male; physical therapy group: mean age 60, 20 female, 3 male) pain intensity was decreased, HFFD and M. Schober were increased, functionality recovered, pain and physical functions of SF-36 were improved after the treatments. SF-36-physical role was also improved in the exercise group. Decrease in pain, increase in HFFD and improving of the functional status were all significantly more in the physical therapy group. There were no difference between the groups in terms of M. Schober measurement and changes of the quality of life. Conclusions: Exercises and exercises+physical therapy are both effective in chronic low back pain. Successful results can be taken by addition of the physical therapy in patients who do not benefit sufficiently from exercise therapy.
