An unnoticed cause of foot pain: tibial sesamoid fracture

Introduction
Hallux sesamoids are composed of two sesamoid bones within the flexor hallucis brevis tendon inferior to the metatarsophalangeal joint of the pollex. The one that is present in medial part is called the tibial sesamoid and the lateral one is called as fibular sesamoid. The tibial sesamoid is more frequently affected by trauma, as it is centrally located to inferior the first metatarsal bone.
Case
A 41-year-old female patient was admitted to the clinic with the complaints of pain in the right sole and around the pollex that had persisted for two weeks. The patient explained that the pain developed after a long walk wearing high heels. She was admitted to center with these complaints, She was admitted to a center with these complaints, diagnosed with hallux valgus and was recommended to receive medical therapy and wear a hallux valgus relaxation brace. Upon physical examination, there was tenderness in first metatarsophalangeal joint of right pollex. The joint movements were markedly painful especially during extension. Hallux valgus deformity was detected. A linear line causing separation in the tibial sesamoid bone of the right pollex was detected in the roentgenogram of the right foot. Magnetic resonance imaging was performed with the initial diagnoses of fracture and bipartite tibial sesamoid and a hypointense fracture line consistent with fracture in the tibial sesamoid bone was detected in T1and T2 weighted images. The patient was consulted with the orthopedics and traumatology department. Metatarsal pad together with foot orthosis and no weight bearing for four weeks were recommended. It was observed that the patient’s complaints regressed after four weeks.
Conclusion
Although a trauma was not present, the possibility ofa tibial sesamoid fracture should be considered in patients who are admitted with the complaints of pollex pain and the patient should be evaluated in this respect.

Giriş
Halluks sesamoidleri; fleksör hallusis brevis tendonunun içinde ayak başparmağın metatarso-falangeal eklemi altında bulunan iki sesamoid kemikten oluşmaktadır. Medialde bulunan Tibial sesamoid, lateraldeki ise Fibular sesamoid olarak adlandırılır. Tibial sesamoid birinci metatarsal kemiğin hemen altında ve santralde yerleşim gösterdiğinden travmalardan daha sık etkilenmektedir.
Olgu sunumu
Kırk bir yaşında bayan hasta, sağ ayak tabanı ve başparmak çevresinde yaklaşık 2 haftadır var olan ağrı yakınmasıyla kliniğimize başvurdu. Hasta ağrı yakınmasının yüksek topuklu bir ayakkabı ile uzun süreli bir yürüyüş sonrası ortaya çıktığını ifade etti. Bu şikâyetlerle başvurduğu merkezde halluks valgus tanısı konulduğu, medikal tedavi ve halluks valgus istirahat ateli verildiği öğrenildi. Fizik muayenede sağ ayak birinci parmak metatarsofalengeal eklemde presyonla hassasiyet mevcuttu. Eklem hareketleri özellikle ekstansiyonda belirgin ağrılı olarak bulundu. Halluks valgus deformitesi gözlendi. Sağ ayak röntgenogramında sağ ayak 1. Parmak Tibial sesamoid kemikte seperasyona neden olan lineer hat izlendi. Fraktür? Bipartite Tibial sesamoid ön tanılarıyla istenen Manyetik Rezonans Görüntülemede (MRG) T1ve T2 ağırlıklı kesitlerde Tibial sesamoid kemikte fraktür ile uyumlu hipointens kırık hattı izlendi.Ortopedi ile konsulte edildi. Metatarsal ped ile birlikte, ayak ortezi önerilerek 4 hafta boyunca yük verilmemesi önerildi. 4 hafta sonra hastanın şikâyetlerinin gerilediği görüldü.
Sonuç
Ayak başparmak ağrısı ile başvuran hastalarda, özellikle Halluks valgus varlığında, belirgin bir travma olmasa da Tibial sesamoid fraktürü olasılığı dikkate alınmalı hasta bu açıdan değerlendirilmelidir
