Radial Vein Thrombosis, After Radial Head Fracture: A Case Report

A 36 years old male, diagnosed with radial head fracture had conservative treatment with long arm cast for one month. After long arm cast was removed, swelling of the dorsum of the hand and forearm pain evolved gradually. A duplex ultrasound revealed an acute thrombus of the right radial vein. After anticoagulant therapy, symptoms decreased significantly.
Upper extremity deep venous thrombosis, although not as common as lower extremity deep venous thrombosis, is a clinical entity with considerable morbidity, may lead to pulmonary embolism and post thrombotic syndrome. Early recognition and appropriate treatment is important to prevent this complications.
Immobilization induced venous stasis is also a risk factor for upper extremity deep venous thrombosis not only for lower extremity. This case report is presented to emphasize that upper extremity deep venous thrombosis should be considered in patients during and after immobilization period due to arm casts.
Radius Başı Fraktürü Sonrasında Radial Ven Trombozu: Olgu Sunumu

Radius başında fraktür sonrası, 1 ay uzun kol sirküler alçı ile konservatif izleme alınan 36 yaşında erkek hastanın, alçı çıktıktan sonra el dorsumunda giderek artan şişlik ve önkol ağrısı gelişmiştir. Üst ekstremite venöz renkli dupleks ultrasonografisi sonucuna göre, sağ radial vende akut süreçte trombüs saptanmıştır. Hastada radial ven trombozu düşünülerek verilen antikoagülan tedavi sonrası semptomları belirgin gerilemiştir.
Üst ekstremite derin ven trombozu, alt ekstremite derin ven trombozları kadar sık görülmemekle birlikte, önemli bir morbidite nedenidir. Pulmoner emboli ve post trombotik sendrom bu morbiditelerin başında gelmektedir. Derin ven trombozunun erken saptanarak ve erken dönemde tedaviye başlanması, bu morbiditelerin önüne geçilmesinde önemlidir.
İmmobilizasyon, genellikle alt ekstremite trombozu için risk faktörü olarak bilinmekle birlikte, virchow triadının komponentlerinden olan staza neden olduğu için üst ekstremite venlerinde de tromboza neden olabilen bir durumdur. Bu olgu, alçı tedavisi sonrası sadece alt ekstremitede değil, immobilizasyon süreci ve sonrasında üst ekstremite de derin ven trombozu gelişebileceğini vurgulamak için sunulmuştur.
