PFAPA Who Benefit From Tonsillectomy: A Case Report

PFAPA syndrome is characterized by sudden onset of high fever, aphthous stomatitis, pharyngitis and cervical lymphadenopathy. This syndrome, which usually observes under 5 years of age and common in boys, has a benign course and with no long-term sequelae. Although clinical findings are clear enough, as there is no specific test for these diseases, it is sometimes hard to diagnose. İn the treatment of PFAPA syndrome, a single dose of oral prednisone relieves symptoms quickly but some cases are unresponsive to steroid therapy. This case report is about a patient with the diagnosis of PFAPA who is unresponsive to steroid therapy and supplied complete recovery with tonsillectomy.

Bir periyodik ateş sebebi olan, PFAPA sendromu ani başlayan yüksek ateş, aftöz stomatit, farenjit ve servikal lenfadenopati ile karakterizedir. Genellikle beş yaşından küçüklerde ve erkeklerde daha sık görülen bu sendrom selim seyirlidir ve uzun dönemde sekel gelişmez. Klinik tablo oldukça iyi tanımlanmasına karşın hastalığa özgü laboratuvar bulgularının olmaması tanıyı güçleştirmektedir. PFAPA’lı çocukların tedavisinde atak başında tek doz oral prednizon (1-2mg/kg) verilmesi semptomları hızlı bir şekilde düzeltmektedir ancak bazı olgularda zamanla beraber steroid tedavisine yanıt alınamamaktadır. Bu olgu sunumunda PFAPA tanısı alan steroid tedavisine yanıt vermeyip tonsillektomi ile tam iyileşme sağlayan hastanın klinik seyri anlatılacaktır.
