Ektopik Gebelik Tedavisinde Tek Doz Methotrexat Başarısının Öngörülmesi

GİRİŞ ve AMAÇ: Ektopik gebelik tedavisinde tek doz methotrexat tedavisinin etkinliğini ve başarıyı etkileyen faktörleri araştırmaktır.
YÖNTEM ve GEREÇLER: Ocak 2013-Aralık 2017 tarihleri arasında kliniğimizde tek doz methotrexat ile tedavi edilen olgular başarılı olanlar ve olmayanlar olarak karşılaştırıldı. Hastaların demografik özellikleri, tedavi başlangıcındaki, 4. ve 7. gün β-HCG değerleri ve ektopik odak boyutu değerlendirildi.
BULGULAR: Ektopik gebelik tanısı alan ve başlangıç tedavisi olarak tek doz metotreksat tedavisi uygulanan toplam 462 olgu çalışmaya dahil edildi. Tek doz metotreksat tedavisi 350 (%75,8) hastada başarılı olurken 112 (%24,2) hastada başarısız oldu. Methotrexat uygulanan gündeki serum ß- hCG değeri, 4. gün ve 7. gün ß- hCG değerleri ve ektopik odak boyutu tek doz methotrexat başarısız olan grupta anlamlı olarak daha yüksek saptandı (p<0,001).
TARTIŞMA ve SONUÇ: Hemodinamisi stabil olan hastalarda ektopik gebelik tedavisinde tek doz methotrexat tedavisinin başarısını etkileyen çok sayıda faktör vardır. Bunlardan en önemlileri ektopik odak boyutu ve başlangıç ß-hCG değeridir.
Prediction of Single Dose Methotrexate Success in Ectopic Pregnancy Treatment

INTRODUCTION: To investigate the efficacy of single dose methotrexate treatment and the factors affecting its success in the treatment of ectopic pregnancy.
METHODS: Ectopic pregnancy cases treated with single dose methotrexate therapy between January 2013 and December 2017 were compared as successful and unsuccessful. Demographic characteristics of patients, and ectopic mass size, β-HCG values at the beginning of treatment, 4th and 7th days were evaluated.
RESULTS: A total of 462 patients who underwent single dose methotrexate therapy as initial ectopic pregnancy treatment were included in the study. Single dose methotrexate therapy was successful in 350 patients (75.8%) and failed in 112 patients (24.2%). The serum ß-hCG value on the day of administration of methotrexate, ß-hCG values on day 4 and day 7, and the ectopic mass size were significantly higher in the failed group (p <0.001).
DISCUSSION AND CONCLUSION: There are a number of factors that affect the success of single dose methotrexate therapy in ectopic pregnancy in hemodynamically stable patients. The most important of these are the ectopic mass size and the initial ß-hCG value.
