Üretra darlığının cerrahi tedavisinde amplatz renal dilatatör kullanımı

Giriş: Üretra darlığı; tanı, tedavi zorluğu, tekrarlaması nedeniyle hem hasta hem de hekim açısından ürolojinin sorunlu konularında birisidir. Tedavisinde birçok yöntemler tanımlansa bile üretra dilatasyonu bilinen en eski yöntemdir.
YÖNTEM ve GEREÇLER: Kliniğimizde üretra darlığı tanısı ile amplatz renal dilatatör ile tedavi edilen 30 hastanın 1 yıllık sonuçları retrospektif olarak incelendi. Tüm hastaların operasyon öncesi ve operasyon sonrası 3,6,9 ve 12. aylardaki maksimum idrar akım hızları (Qmax) ve operasyon sonrası 12. ayda Uluslararası Prostat Semptom Skoru (IPSS) sonuçları kaydedildi.
BULGULAR: Hastaların yaş ortalaması 61.6 ± 6.7 olarak bulundu. Operasyon öncesi maksimum idrar akım hızı 7,6±1,5 mL/sn iken operasyon sonrası 3, 6, 9 ve 12. aylarda sıra ile 16,1±2,3, 16,1±1,6, 15,7±1,7 ve 15,1±2,2 olarak bulundu. Operasyon öncesi IPSS skoru 18.4 ± 2.4 iken oprasyon sonrası 12. ayda 9,7±2,7 olarak bulundu. Sadece 3 hastada (%10) nüks izlendi.
TARTIŞMA ve SONUÇ: Üretra darlığı tedavisinde amlatz dilatasyon yöntemi basit, etkili ve güvenli bir yöntemdir ve klasik İÜ yöntemine alternatif bir tekniktir.
Use of amplatz renal dilator for surgical treatment of urethral strictures

Introduction: Urethral stricture is one of the complex issues of urology due to the difficulty of diagnosis, treatment and risk of recurrence. Many procedures are available for the treatment of urethral strictures; urethral dilatation is one of the oldest.
METHODS: A total of 26 men with urethral strictures were managed by urethral dilatation using amplatz renal dilators. The parameters analyzed included presentation of patients, pre-and postoperative maximum flow rate (Qmax) on uroflowmetry (UF) and International Erectile Function Index-5. Patients were followed-up at 3, 9 and 12 months.
RESULTS: The mean of age was 61.6 ± 6.7. While the mean preoperative Qmax value was 7,6±1,5 mL/sec., the mean postoperative Qmax values were 16,1±2,3, 16,1±1,6, 15,7±1,7 and 15,1±2,2 in 3,6 and 12 months respectively.
DISCUSSION AND CONCLUSION: This tecnique is a safe and effective alternative for treatment of urethral strictures.
