Çölyak Hastalarında Hematolojik Parametrelerin Hasta Takibi Ve Diyet Uyumu İle Olan İlişkisi

GİRİŞ ve AMAÇ: Çölyak hastalığı genetik olarak duyarlı kişilerde başlıca buğday, arpa, çavdar, yulaf gibi tahıllardaki gluten ve gluten benzeri diğer tahıl proteinlerine karşı kalıcı intolerans olarak gelişen proksimal ince barsak hastalığıdır. Tedavi ömür boyu sürecek glutensiz diyettir. Bu tedaviye sıkı bir şekilde uyulması hastalığın prognozu açısından önemlidir. Hastalığın takibi oldukça maliyetli olduğundan diyet uyumu ve antikor düzeyini yansıtabilecek daha kolay uygulanabilir ve ek maliyet gerektirmeyen parametrelere ihtiyaç vardır. Bu çalışmamızda Çölyak hastalarında hemogram parametreleri ve albümin düzeyinin glutensiz diyete uyumu değerlendirmede kullanılıp kullanılamayacağını belirlemeyi amaçladık.
YÖNTEM ve GEREÇLER: 133 hastadan, Çölyak hastalığı tanısı Anti-dTG-IgA düzeyleri ile konfirme olan 57 hasta çalışmaya dahil edildi. Hastaların Anti-dTG-IgA düzeylerinin pozitif ve negatif olduğu dönemlerdeki NLR, PLR, MPV, PDW, RDW ve albümin düzeyleri karşılaştırıldı ve glutensiz diyete uyumları ile ilişkisi değerlendirildi.
BULGULAR: Hastaların ortalama yaşı 38,96(19-66) idi. Anti-dTG-IgA pozitif olduğu dönemde negatif olduğu döneme kıyasla RDW değeri(p=0,005) istatistiksel olarak anlamlı derecede yüksekti. PDW(p=0,02) ve albümin(p=0,035) değerleri ise anlamlı olarak düşük bulundu. PLR(p=0,074) değeri daha yüksek saptansa da bu fark istatistiksel olarak anlamlılık sınırını aşamadı. NLR(p=0,69) ve MPV(p=0,12) değerlerinde ise fark saptanmadı.
TARTIŞMA ve SONUÇ: Çölyak hastalarının diyet uyumunu ve antikor düzeylerini değerlendirmede daha az maliyetli olan ve PDW, RDW ve albümin düzeyleri hasta takibine yardımcı birer parametre olarak kullanılabilir.
Relationship Between Hematological Parameters and Followup and Diet Compliance in Celiac Patients

INTRODUCTION: Celiac disease is a proximal small bowel disease developing due to persistent intolerance in people who are genetically susceptible to gluten and other gluten-like grain proteins in cereals such as mainly wheat, barley, rye, oats. Treatment is a lifelong gluten-free diet. Strict adherence to this treatment is important for the prognosis of the disease. Since it is quite costly to follow-up of the disease, there is need to parameters that are easy to apply to reflect diet compliance and antibody level and do not require. In this study we aimed to determine whether hemogram parameters and albumin level can be used to evaluate the compliance of gluten-free diet in the celiac patients.
METHODS: 57 of 133 Celiac patients whose disease was confirmed with Anti-dTG-IgA levels were included to the study. NLR, PLR, MPV, PDW, RDW and albumin levels were compared and the relationship between adherence to a gluten- free diet and adherence to a gluten-free diet was evaluated in periods when Anti-tTG-IgA levels are positive and negative.
RESULTS: The mean age of the patients was 38.96(19-66). RDW value, at Anti-tTG-IgA positive the period was significantly higher compared to the period when it was negative(p=0.005). PDW(p=0.02) and albumin(p=0.035) values were significantly low. Although the PLR(p=0.074) value was found to be higher, this difference was not statistically significant. There was no difference in NLR(p=0,69) and MPV(p=0,12) values.
DISCUSSION AND CONCLUSION: PDW, RDW, albumin levels are more cost-effective, and they can be used as an auxiliary parameter to evaluate dietary adherence and antibody levels in Celiac patients’ follow-up.
