Anoftalmik sokete bağlı üst göz kapağı sulkus deformitesinin dermis yağ grefti ile volumetrik düzeltilmesi
Mehmet Serhat ManganHaydarpaşa Numune Eğitim ve Araştırma Hastanesi, Sadık Eratik Göz Kliniği, İstanbul, TürkiyeAnoftalmik soket hastalarında orbital dokuların zamanla fibrozisine bağlı enoftalmi, sığ forniks ve derin üst göz kapağı sulkusu oluşabilmektedir. 42 yaşında kadın hasta polikliniğimize sağ göz üst kapaktaki çökük görünüşten memnuniyetsizliği nedeniyle başvurmuştu. Daha öncesinde evisserasyon cerrahisi uygulanan ve yaklaşık 30 yıldır göz protezi kullanma öyküsü mevcut olan hastanın göz muayenesinde sağ göz üst göz kapağında derin sulkus deformitesi mevcuttu. Hastaya dermis yağ grefti ile üst göz kapağı sulkus deformitesi onarımı uygulandı. Ameliyat sonrası kontrol muayenesinde üst göz kapağı sulkus deformitesinin belirgin şekilde düzeldiği izlendi. Sonuç olarak, anoftalmik soket hastalarında gelişen intrakonal veya ekstrakonal hacim kaybını düzeltmek için dermis yağ greftleri alternatif ve güvenilir bir yöntem olarak kullanılabilir.

Mehmet Serhat ManganHaydarpasa Numune Education and Research Hospital, Sadik Eratik Eye Clinic, İstanbul, TurkeyEnophthalmos, a shallow fornix, and a deep superior sulcus in the eyelid may be encountered in patients with an anophthalmic socket due to the gradual fibrosis of the orbital tissues. A 42-year-old female presented to our clinic because of dissatisfaction with the sunken appearance of the right upper eyelid. She had undergone evisceration surgery in the past and had been using a prosthesis in the right eye for the last 30 years. The ophthalmic examination revealed a deep superior sulcus deformity of the right eyelid. A dermis fat graft was placed together with volumetric correction of the right upper eyelid sulcus deformity. There was significant rejuvenation of the upper eyelid sulcus deformity on the postoperative follow-up examination. In conclusion, dermis fat grafts can be used as a safe alternative method for volumetric correction of the intraconal or extraconal volume loss that develops in patients with anophthalmic socket.
