BPH nedeniyle Bipolar TURP ve Monopolar TURP Yapılan Hastaların Karşılaştırılması

GİRİŞ ve AMAÇ: BPH dünyada en yaygın görülen hastalıklardan biridir.TURP(Transurethral resection of Prostate) BPH cerrahisinde altın standart yöntemdir.TURP yöntemleri bu yüzden önemlidir.Çalışmamızdaki amaç Bipolar TURP ve Monopolar TURP yöntemlerini karşılaştırmaktır.
YÖNTEM ve GEREÇLER: Çalışmamıza BPH nedeniyle TURP yapılan 120 hasta dahil edildi. Hastalar 60 kişilik 2 grup olarak incelendi.Grubun birine Monopolar TURP uygulanırken diğer gruba Bipolar TURP uygulandı. Hastaların IPSS,üroflowmetri, hemoglobin kaybı,postop kateterizasyon süresi, hastane kalış süresi,TUR sendromu ve postop 6. aydaki üretral striktür parametreleri incelenmiştir. İstatiksel analizde ki-kare,Student T test ve Wilcoxon testi uygulandı.
BULGULAR: İki grup ayrı ayrı incelendi ve karşılaştırıldı. IPSS,üroflowmetri,hastanede kalış süresi,hemoglobin kaybı parametrelerinde istatistiksel olarak anlamlı fark saptanmamıştır(p>0,05). Postop kateterizasyon süresi ve postop 6. ayda üretral striktür gelişimi istatiksel olarak anlamlı saptanmış; M-TURP grubunda daha çok striktür geliştiği ve kateterizasyon süresinin daha uzun olduğu gözlemlenmiştir(p<0,05). Aynı zamanda M-TURP grubunda 2 hastada TUR sendromu gelişirken B-TURP grubunda TUR sendromu gözlenmemiştir (p<0,05).
TARTIŞMA ve SONUÇ: Bipolar TURP ve Monopolar TURP grubu karşılaştırıldı.Her iki grup sonuçları benzer olarak olarak bulundu.Sadece Bipolar TURP grubunda kateterizasyon süresi,striktür olasılığı ve TUR sendromu gelişimi açısından daha avantajlı bulundu.Sonuç olarak Bipolar TURP Monopolar TURP ye göre daha iyi bir yöntem olsada her iki yöntem de BPH cerrahisinde güvenilir olarak uygulanabilir.
Comparison of Bipolar TURP and Monopolar TURP Patients who underwent surgery Due to BPH.

INTRODUCTION: BPH is the one of the most common disease in the world. TURP (Transurethral resection of prostate) is the gold standard procedure in BPH surgery.So TURP methods are important. The aim of our study is to compare Bipolar TURP and Monopolar TURP methods.
METHODS: In our study is included 120 patients who underwent TURP due to BPH. The patients were examined in two groups of 60 people. Monopoly TURP was applied to one group while Bipolar TURP was applied to the other group. Patients IPSS, uroflowmetry, hemoglobin loss, postoperative catheterization time, hospital stay, TUR syndrome and 6th month postoperative urethral stricture parameters were investigated. In the statistical analysis, Chi-Square, Student T test and Wilcoxon test were performed.
RESULTS: Two groups were analyzed separately and compared.In IPSS, uroflowmetry, hospital stay, hemoglobin loss parameters there was no significantly difference between two groups.The duration of postoperative catheterization and development of urethral stricture at postoperative 6th months found statistically significant; In the M-TURP group, more stricture development and longer catheterization time were observed (p <0.05).At the same time TUR syndrome was observed in 2 patients in M-TURP group and TUR syndrome was not observed in B-TURP group (p <0,05).
DISCUSSION AND CONCLUSION: Bipolar TURP and Monopolar TURP groups were compared.The results of both groups were similar.Only in the Bipolar TURP group, in terms of the probability of stricture and the development of the TUR syndrome was more advantageous than Monopolar TURP. In conclusion, Bipolar TURP is more reliable method than Monopolar TURP and both methods can be reliably applied in BPH surgery.
