Yoğun bakım ünitelerinde el yıkama davranışlarının incelenmesi

GİRİŞ ve AMAÇ: Amaç: Uygun teknikle el hijyeni kurallarına uyum sağlık bakımı ilişkili enfeksiyonların önlenmesinde en kolay ve etkili yöntemlerden biridir. Araştırma yoğun bakım ünitelerinde el yıkama davranışlarını incelemek amacı ile gerçekleştirildi.
YÖNTEM ve GEREÇLER: Retrospektif ve tanımlayıcı tipteki araştırmanın verileri Ocak 2017-Nisan 2018 tarihleri arasında hastanenin enfeksiyon kontrol komitesi birimi tarafından yoğun bakım ünitelerinde 09.00-12.00 saatleri arasında el yıkama davranışlarını gözlemleyerek “El Hijyeni Gözlem Formu”na kayıt ettikleri bilgilerden elde edildi.
BULGULAR: Toplam 924 el yıkamanın gözlemlendiği araştırmada en fazla el yıkayan sağlık ekibi üyesinin hemşireler (n=572, %61,90) olduğu; 5 El Yıkama Endikasyonuna göre en fazla hasta ile temas sonrası (n=337, %36,5) el yıkamanın yapıldığı; aseptik işlemler öncesi el yıkama oranının hemşirelerde (n=86, %9,31) diğer sağlık ekibine (n=19, %2,06) oranla istatistiksel anlamlılıkla daha yüksek olduğu görüldü (p<0,05).
TARTIŞMA ve SONUÇ: İncelenen yoğun bakım ünitelerinde 5 El Yıkama Endikasyonuna göre el yıkama oranlarının istendik düzeyde olmadığı görüldü. Bu ünitelerde el yıkamaya engel oluşturan durumların araştırılarak el hijyenine uyum programlarının geliştirilmesi ve sürdürülmesi önerilmektedir.
Investigation of hand washing behaviors in intensive care units

INTRODUCTION: Following the rules of hand hygiene with the proper technique is one of the easiest and most efficient way of preventing health care related infections. The research was conducted with the aim of investigating the hand washing behaviours in the intensive care units.
METHODS: Data for the retrospective and descriptive type of research was collected between January 2017-April 2018 by the infection control unit of the hospital by observing the hand washing behaviours at the intensive care units between 09.00-12.00 and recording these observations to the “Hand Hygiene Observation Form”.
RESULTS: A total of 924 hand washes were observed for the research and it was detected that nurses were the health team members who washed hands the most (n=572, 61.90%); hand washing was performed the most after making contact with the patients, according to the 5 Hand Washing Indications (n=337, 36.5%); hand washing rate before aseptic operations was significantly higher among nurses (n=86, 9.31%) compared to other health teams (n=19, 2.06%) (p<0.05).
DISCUSSION AND CONCLUSION: It was identified that hand washing rates at the intensive care units were unsatisfactory according to the 5 Hand Washing Indications. It is recommended that barriers to handwashing in these units should be investigated and programs to adjust to hand hygiene should be developed and maintained.
