GİRİŞ ve AMAÇ: Covid-19 ilişkili akut respiratuar distres sendromu (ARDS) hastalarında komplians açısından ARDS fenotipleri tanımlanmıştır. Çalışmamızda Covid-19 ilişkili ARDS hastalarında mekanik ventilasyon desteği ve sonuçları incelenmiştir. YÖNTEM ve GEREÇLER: Retrospektif planlanan çalışma, hesaplanan statik komplians değerine göre üç fenotip alt grup belirlenen Covid-19 ilişkili ARDS hastalarında gerçekleştirildi (Tip H = Cstat < 40 ml/cm H2O, Tip L = Cstat ≥ 50 ml/cm H2O ve Tip Int = 40 ≤ Cstat < 50 ml/cm H2O). Mekanik ventilasyon stratejisinin Covid-19 ARDS fenotiplerinde, mekanik ventilasyon süresi, yoğun bakım ünitesinde kalış süresi ve mortalite üzerine etkileri incelendi. BULGULAR: Çalışma hastalarının Covid-19 ARDS fenotipi açısından % 72.3 Tip H, % 23.4 Tip L, % 4.3 Tip Int olduğu tespit edildi. Mekanik ventilasyon stratejisinin, mekanik ventilasyon süresi (p = 0.357), yoğun bakım ünitesinde kalış süresi (p = 0.127) ve ölüm oranını (p = 0.583) etkilemediği görüldü. TARTIŞMA ve SONUÇ: Covid-19 ilişkili ARDS hastalarında, komplians ilişkili fenotipten bağımsız koruyucu ventilasyon stratejisinin uygun olduğunu düşünüyoruz.

INTRODUCTION: Covid-19-related acute respiratory distress syndrome (ARDS) phenotypes in terms of lung compliance have been described in patients with Covid-19-related ARDS. In our study, mechanical ventilation support and its results were examined in patients with Covid-19-related ARDS. METHODS: The retrospective study was carried out in patients with ARDS associated with Covid-19 by determining three phenotype subgroups according to the calculated static compliance value. Type H (High) = Cstat < 40 ml/cm H2O, Type L (Light) = Cstat ≥ 50 ml/cm H2O in terms of ARDS phenotype and Type Int (Intermediate) = 40 ≤ Cstat < 50 ml/cm H2O. The effects of mechanical ventilation strategy on Covid-19 ARDS phenotypes, duration of mechanical ventilation, duration of intensive care unit and mortality were examined.
RESULTS: Study patients were found to be 72.3 % Type H, 23.4 % Type L, 4.3 % Type Int in terms of Covid-19 ARDS phenotype. Mechanical ventilation strategy did not affect the duration of mechanical ventilation (p = 0.357), duration of intensive care unit) (p = 0.127), and mortality rate (p = 0.583) in all three phenotypes. DISCUSSION AND CONCLUSION: We believe that a phenotype-independent protective ventilation strategy defined according to compliance is appropriate in patients with Covid-19-associated ARDS.
