Hemşirelerin Yaşam Sonu Bakıma Yönelik Tutum ve Davranışlarının Klinik Karar Vermeye Etkisi

YÖNTEM ve GEREÇLER: Tanımlayıcı ve kesitsel tipteki bu araştırma Nisan 2019 -Ağustos 2019 tarihleri arasında, bir eğitim ve araştırma hastanesinin yoğun bakım ünitelerinde çalışan 84 hemşire ile gerçekleştirilmiştir. Veriler, “Birey Tanıtım Formu", "Yoğun Bakım Hemşirelerinin Yaşam Sonu Bakıma Yönelik Tutum ve Davranışları Ölçeği” ve “Hemşirelikte Klinik Karar Verme Ölçeği” kullanılarak toplanmıştır.
BULGULAR: Araştırmaya katılan yoğun bakım hemşirelerinin yaş ortalaması 33.13±5.79 olup, %89.3’ünün kadın, %64.3’ün lisans mezunudur. Yoğun Bakım Hemşirelerinin Yaşam Sonu Bakıma Yönelik Tutum ve Davranışları Ölçeği toplam puan ortalaması 57.91±10.53, Hemşirelikte Klinik Karar Verme Ölçeği toplam puan ortalaması 140.64±23.52 olarak belirlenmiştir. Yüksek lisans eğitim düzeyine sahip hemşirelerin Yoğun Bakım Hemşirelerinin Yaşam Sonu Bakıma Yönelik Tutum ve Davranışları Ölçeği toplam puan ortalamasının istatistiksel olarak anlamlı şekilde daha yüksek olduğu bulunmuştur. Ayrıca yaşam sonu bakıma yönelik eğitim alan hemşirelerin Hemşirelikte Klinik Karar Verme Ölçeği toplam puan ortalamasının daha yüksek olduğu saptanmıştır. Yoğun bakım hemşirelerinin Yoğun Bakım Hemşirelerinin Yaşam Sonu Bakıma Yönelik Tutum ve Davranışları Ölçeği puan ortalaması ile Hemşirelikte Klinik Karar Verme Ölçeği puan ortalaması arasında pozitif yönde düşük düzeyde ilişki olduğu saptanmıştır.
TARTIŞMA ve SONUÇ: Hemşirelerin yaşam sonu bakıma yönelik tutum ve davranışları ile klinik karar verme düzeyi arasında düşük düzeyde ilişki olduğu ve hemşirelerin yaşam sonu bakıma yönelik tutum ve davranışlarının klinik karar verme düzeyini etkilediği söylenebilir.
The Effect of Nurses' Attitudes and Behaviors Towards End-of-Life Care on Clinical Decision Making

METHODS: This descriptive and cross-sectional study was conducted between April 2019 and August 2019 with 84 nurses working in intensive care units of a training and research hospital. Data were collected using the “Individual Introduction Form”, “the Scale of the Attitudes and Behaviors of Intensive Care Nurses Intended for End of Life Care” and “Clinical Decision Making Scale in Nursing”.
RESULTS: The mean age of the intensive care nurses participating in the study was 33.13 ± 5.79. It was determined that 89.3% of them were female and 64.3% were undergraduate. It was determined that the total mean score of “the Scale of the Attitudes and Behaviors of Intensive Care Nurses Intended for End of Life Care” was 57.91±10.53 and the total mean score of “the Clinical Decision Making in Nursing Scale” was 140.64 ± 23.52. It was found that the total mean score of the Scale of the Attitudes and Behaviors of Intensive Care Nurses Intended for End of Life Care of the nurses with master’s education level was significantly higher. In addition, it was found that the total mean score of the Clinical Decision Making in Nursing Scale of nurses who received training for end-of-life care was higher. It was determined that there was a positive low level relationship between the Scale of The Attitudes and Behaviors of Intensive Care Nurses Intended for End of Life Care and the mean score of the Clinical Decision Making in Nursing Scale.
DISCUSSION AND CONCLUSION: It can be said that there was a low level of relationship between nurses’ attitudes and behaviors towards end-of-life care and clinical decision-making level, and that as their attitudes and behaviors towards end-of-life care affect the clinical decision-making level.
